It may have been all they had. Have you tried phoning the Waltham city
clerk or the Historical Society? If you don't want a certified copy then
they may give you the info over the phone. Every town/city is different
[info kept, how they recorded it, and what they will send you] when it
comes to vital records. I know that my town [Boylston] doesn't issue
"certificates" but a form that looks like one will be filled out if you
want[that form may not contain ALL the info the town has tho!] You can
call the clerk and get info over the phone and she will send you what info
they do have. That info is different for the various years - some are
scant others have alot of info on parents etc.
