Don't know if you have received an answer to your request but you can find
death notices and obituaries from the Boston Public Library.

They are very helpful and will search several newspapers for you. It is very
inexpensive. There is a limit of three people per request and with handling it
was about $8.50. There is no charge until they find something.
The format is as you state, depending on who wrote it.
