Working on indexing some Ocean County, NJ Declaration of Intent pages for the
Historical Society and I have found that the form indicates the Petitioners
name [sometime former name also], Current Residence, usually just the town,
Date and place of birth, date it was recorded, country of origin, country
emmigrated from, Occupation, name of spouse, and some list names of children
