Hi June,
They still have the old form, but for some reason it is not on their
website. The last time I ordered records from them I crossed off what was
on the form and wrote in the information I had. When they sent it back to
me they sent me a copy of the correct form. I scanned it and now have it on
my computer.
If you would like I can send you a copy in an Adobe Acrobat format.
